You can be as relaxed as you wish.

 

If its mostly hot cold.......paint lots of LinerBond2V
around all the margins....360degrees...
use the dual cure...so it sets whether you get light there or not..
 
LB2V is the best desensitizer I have ever used...
4Meta is also pretty good but more complicated..
 
when I'm not so serious....we use Superseal...
and we are now playing with Biflurid from Voco...
 
What pain tablets?   Ibuprofen?....I hope...
you need something anti-inflamatory...
 
On her own...tell her to apply Listerine on the end
of a OralB or Butler rubber tip...or sulcabrush...
or something tiny and pointy that will act as a vehicle
for the listerine to get there...
again...listerine does kill plaque ...when you put it where the plaque is..
not when you rinse with it and it goes everywhere but where you need it..
(sort of subgingival irrigation or atleast application)
The essential oils...Eucalyptus Thymol and Menthol....
are anti-inflamatory....hence reversing gingivitis...
also only if you put it where it needs to be...
 
Occlusal adjustment....desensitization.....Advil....and Listerine
almost always work...
unfortunately it cuts down on a lot of elective endos...
 
remember there could also be a systemic bacteremia...
and the pulp could be transiently inflamed..
and then resolve again without intervention...
but its always nice to do ...something....even if its paliative..
 
my phylosophy....try the simplest and most non-invasive thing first...
get more aggressive if it doesn't work...
 
extraction...is the ultimate occlusal adjustment...
crown preps are a close second...
endo is a last resort....
if its CTS....and there is a frature...
endo will get rid of H/C pain....but not biting pain...
if the root is still cracked...
 
but on the other hand...cracked roots....which communicate
with odontoblastic processes....in dentin tubules..
cause ingresses of bacteria...and movement of tubular fluid...
which sets up pulpal inflamation...
 
if you can stop the crack from propagating in a vital tooth...
and use a little anti-inflamatory...and keep the plaque away from
any openings like exposed roots.....you get healing...
 
if not...you may get so much inflamation and swelling in the pulp
that it basically chokes off its own blood supply and becomes necrotic
without ever having a lesion reach the pulp.
